Search and contact local resources in your neighbourhood serving older adults, including people living with dementia, and adults with physical disabilities. Services available incluide: meals, homemaking, transportation, exercise classes, and social supports. You can also contact a navigator who can help you to find the services and supports you need.

Assists you in finding a primary care provider.

A free, secure and confidential service you can call or access online 24 hours a day, seven days a week, to get health advice from a registered nurse or find health services or information.

A free and confidential service that can be accessed 24 hours a day, in 150 languages, by phone, text and web. Provides information and navigation for government and community-based, non-clinical health and social services.

Provides coordinated access to mental health services and supports across Ontario.
Ontario Caregiver Organization
Supports caregivers by being their one point of access to information, so they have what they need to be successful in their role.
Ontario Health atHome
Coordinates local home and community care, long-term care placement, and helps with finding services in the community. Funded by the Ministry of Health.

Provides support, education and connecting care and services for new immigrants.

Provides resources for stroke care and recovery, and peer support connections.

Provides a wide variety of activities, programs, services, support groups as well as educational resources for anyone feeling the impact of dementia.
