Geriatric Assessment Outreach Team
The Geriatric Assessment Outreach Team based at Bruyère Health Saint-Vincent Hospital provides comprehensive multidimensional screening assessments for seniors with complex needs, in their own homes. This interdisciplinary team of professionals serves as a primary point of access for community referrals to the Regional Geriatric Assessment Program, a network of specialized geriatric services designed to avoid or reduce preventable losses of health and independence associated with aging.

What we do?
A Geriatric Assessor completes a comprehensive geriatric assessment on adults who are 65 years or older.
Our Goals
- Improve quality of life.
- Optimize functional independence, autonomy, health and safety in the community.
- Prevent and avoid unnecessary emergency department visits and hospitalizations.
Older adults are referred by their Primary Care Provider, other healthcare professionals, family, caregivers or themselves. Referrals are accepted for people living in the community. Referrals cannot be accepted from long-term care facilities or out of province.
Our Outreach Teams are committed to working with the Primary Care Provider and community services agencies.
